An exciting opportunity has arisen for a Band 6 Speech and Language Therapist to work in the community CPFT Speech and Language Therapy Team. The post holder will need to be qualified to work independently with patients with dysphagia.

The post holder will work alongside colleagues, providing input to patients with acquired communication and swallowing difficulties. The successful applicant will join a friendly, innovative team of therapists and clinical assistants. The team meet regularly for meetings and the post holder will receive regular line management and clinical supervision.

The post holder will work independently, providing speech and language therapy input to patients on an outpatient and domiciliary basis and input to the community hospital wards, providing a high standard of quality clinical care.

The post will involve working with patients with communication impairment and dysphagia resulting from a wide range of medical conditions.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. • To independently assess, differentiate diagnose and develop and implement specialist care programmes for patients with Speech, Language and swallowing needs in the in-patient/day rehab/outpatient and domiciliary settings
• To work effectively within the integrated care team structure and liaise appropriately with members of the MDT and other agencies.
• To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
• To contribute to the prioritisation and triage of patients within clinical pathways.
• To provide specialist advice to all members of the multi-disciplinary team, service users and appropriate others, of changes involving current care plans, patients' progress and other relevant matters that pertain to the care of the patient.
• To work with clinical assistants within the team delegating work in line with clinical competencies and providing clinical supervision and support for assistants.

• Degree or equivalent in Speech and Language Therapy
• Post graduate dysphagia trained and fully independent in dysphagia management
• Registered Member of HCPC
• Evidence of successful completion of post graduate short specialist courses

• Experience of working with adults with a wide range of acquired communication and swallowing disorders in an inpatient/outpatient setting.
• Experience of working with multi-disciplinary team
• Able to work independently with patients with dysphagia
• Experience of AAC - low and high tech
• Experience of working with patients with progressive neurological disorders
• Understanding of the principles of the Mental Capacity Act

• Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
