South East England Adult Community Services £375 Per Day Ongoing Project

We are recruiting for an experienced Speech and Language Therapist to join an exciting project within Adult Community Services across the South East of England.

This is an excellent opportunity for an SLT with experience in community neuro rehabilitation and Early Supported Discharge (ESD) services, looking for a rewarding role supporting adults within their own homes and local communities.

The Role

The successful candidate will work as part of a multidisciplinary community team, supporting adults with neurological conditions following hospital discharge and throughout their rehabilitation journey.

You will be responsible for delivering high-quality assessment, intervention, and management of communication and swallowing difficulties, helping individuals maximise their independence and achieve meaningful rehabilitation goals within the community.

Key Responsibilities
  • Manage a caseload of adults within community neuro rehabilitation and Early Supported Discharge services
  • Assess, diagnose, and treat communication and swallowing disorders
  • Develop and implement person-centred rehabilitation programmes
  • Support patients transitioning from hospital to community-based care
  • Work collaboratively with multidisciplinary teams, including Occupational Therapists, Physiotherapists, Nurses, and other healthcare professionals
  • Provide education, support, and advice to patients, families, and carers
  • Maintain accurate clinical records and reports in line with professional standards
Essential Requirements
  • Degree or Diploma in Speech and Language Therapy
  • Current HCPC registration
  • Experience working within adult community services, neuro rehabilitation, or Early Supported Discharge pathways
  • Experience managing communication and/or dysphagia needs within adult populations
  • Strong assessment, clinical reasoning, and rehabilitation skills
  • Excellent communication and multidisciplinary working abilities
  • Full UK driving licence and access to a vehicle - essential
